Child Forms

Thank you for choosing Washtenaw County Quality Preschool Partnership Programs, a collaboration of school districts and community-based providers who offer state (GSRP) and federal (Head Start/Early Head Start) funded preschool services.

Whether you are a new or returning family, many important forms are available here to be completed and attached in your family's individualized ChildPlus secure link or submitted to your preferred program by the specified time frames to ensure your child has a complete application. If you have any questions you may reach out to Julie Simpson for further assistance.

Please note: All forms need to be downloaded in order to complete and sign. Signature areas will not show up on a web browser.
